It was introduced into medical use by surgeon Heinrich Braun.

Prior to the discovery of amylocaine and procaine, kokain was a commonly used local anesthetic.[6] Einhorn wished his new discovery to be used for amputations, but for this surgeons preferred general anesthesia. Dentists, however, found it very useful.[7]

What is procaine?

In the past, procaine was part of the composition of pharmaceutical formulations suitable for both parenteral and topical application. However, as of today,  commercially available procaine-based medicines are only for topical use (skin cream, ear drops, dental solution, and dental drops). Like the other active ingredients with this kind of properties, procaine is able to accomplish its action by interacting with the voltage-dependent sodium channels present on the neuronal cell membrane of peripheral nerves.

Through this interaction, procaine inhibits the permeability of the cell membrane to sodium, blocking its entry into the nerve cell. As a result, nerve cell excitation and transmission of the pain stimulus is prevented. in addition, this agent increases electrical excitation threshold, reduces rate of rise of action potential and slows nerve impulse propagation thereby causing loss of sensation. The receptor site is thought to be located at the cytoplasmic (inner) portion of the sodium channel. Procaine has also been shown to bind or antagonize the function of N-methyl-D-aspartate (NMDA) receptors as well as nicotinic acetylcholine receptors and the serotonin receptor-ion channel complex.

The primary use for procaine is as an anaesthetic.

Aside from its use as a dental anesthetic, procaine is used less frequently today, since more effective (and hypoallergenic) alternatives such as lidocaine (Xylocaine) exist. Like other local anesthetics (such as mepivacaine, og prilocaine),

procaine is a vasodilator, thus is often coadministered with epinephrine for the purpose of vasoconstriction. Vasoconstriction helps to reduce bleeding, increases the duration and quality of anesthesia, prevents the drug from reaching systemic circulation in large amounts, and overall reduces the amount of anesthetic required.[8] As a dental anesthesic, for example, more novocaine is needed for root canal treatment than for a simple filling.[1] Unlike kokain, a vasoconstrictor, procaine does not have the euphoric and addictive qualities that put it at risk for abuse.
